Summary

Collaborates with medical providers and other members of the care team to assure coordination of care. Serves as an independent mental health practitioner diagnosing and treating mental health illnesses. Provides brief interventions for patients in crisis. Provides intensive care management for patients with acute or chronic complex needs. In collaboration with the clinical leader and physician leader, provides direction and support to assigned pod. Provides education for residents and staff about behavioral health issues.

Education

Masters level education in Social Work, Marriage and Family Therapy, or Clinical Mental Health Counseling. Masters level education in Social Work.

Certification, Registration & Licensure

At the Family Health Center: Concord: Licensed Independent Social Worker, Licensed Marriage and Family Therapist, Licensed Clinical Mental Health Counselor or Licensed Psychologist required; or required to obtain license within three years of hire date.

At the Family Health Center: Hillsboro-Deering, Licensed Independent Social Worker, Licensed Psychologist required.

Experience

Two years experience in a medical setting is preferred.

Responsibilities
  • Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
  • Provides care management for patients with psychosocial and/or complex health needs with a goal of preventing Emergency Department use and hospitalization. Provides longer-term therapy as appropriate.
  • Monitors schedule in order to maximize patient access.
  • In collaboration with the nurse clinical leader and physician leader, serves as a resource for an assigned pod or patient population.
  • Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
  • Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.
