Typically serving patients aged 65 and above who face challenges in attending traditional medical appointments, the program primarily assists individuals managing multiple chronic conditions, coping with changes in functional ability, handling polypharmacy, and dealing with frailty or debility.

The primary care services provided through this program involve intensive patient management and meticulous care coordination, with a particular focus on advance care planning. To ensure optimal patient outcomes, the program team establishes robust partnerships with various healthcare entities, including inpatient teams, hospice agencies, skilled nursing facilities, physical therapy, occupational therapy, mental health services, and county support services.

Job Description

Provides safe and effective care to ambulatory patients under the supervision of a collaborating physician. Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for the Commonwealth of Virginia and delineated in the written Scope of Practice/Protocol for Nurse Practitioner.

Teamwork/Coordination of Care: Demonstrates ability to communicate effectively and work collaboratively with others. Exhibits respect for and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes. Evidence-Based

Experience

• Minimum 3 year as Nurse Practitioner

Licensure, Education, and Certifications

• State License

• Valid DEA

• Graduate from an accredited program

Minimum Requirements:

  • Certification - Basic Life Support upon start; Advanced Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon start
  • Licensure - Licensed Physician Assistant or Nurse Practitioner; If Nurse Practitioner: RN Compact License(depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a "Practice Agreement" signed by sponsoring/ supervising physician(s), AND a written "Scope of Practice/Protocol" signed by the supervising physician, AND "Drug Enforcement Administration License" (if prescribing controlled substances)
  • Experience - Prior experience required;
  • Education - For Nurse Practitioners: Bachelor's Degree in Nursing and Master's Degree in Advanced Clinical Practice or graduate of a post-masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing; For Physician Assistants: Graduate of an accredited Physician Assistant program
