Concord Hospital Medical Group seeks an experienced Advanced Practice Registered Nurse (APRN) or Physician's Assistant (PA-C) to care for patients through our Geriatric Program. The primary location to see patients will be in one nursing facility. However, additional settings may include long-term care facilities, home visits, or offices.

Patient care is provided under the supervision of a CHMG physician for the Physician's Assistant and in collaboration with a CHMG physician for the Advanced Practice Registered Nurse.

We are looking for someone who is collaborative, adaptable, can work independently, and be part of a team. This is a 30-hour paid/24-hour patient care position. No panel requirement.


Completion of Master's degree in Advanced Practice Nursing or Physician's Assistant.

Certification, Registration & Licensure

Licensure required: NH License as APRN or Physician's Assistant, Current DEA License


Prior primary care is required. Experience in nursing home preferred.

  • Obtains patient health history, performs physical examination, and develops a relevant differential diagnosis.
  • Orders, interprets, and follows up on diagnostic tests.
  • Diagnoses acute and chronic medical problems within the scope of practice, or formulates a comprehensive differential diagnosis.
  • Prescribes and manages medications within the practice standards.
  • Provides individualized patient education related to preventive care according to guidelines.
  • Meets productivity standard of the practice/department.
  • Documents care provided in the medical record according to department standards.
